Our Team are trained professionals that can support clients and their families through this time. Our Nurses will assess your condition and circumstances and develop a care plan which creates a calm and safe environment. Our aim is to reduce disruption to your routine.

There are a collection of illnesses and diseases that can produce the symptoms of dementia. Dementia is described as an age related illness but research has shown that individuals as young as 50 have been diagnosed. Early onset of dementia has been associated with strong family history, which can also be described as cognitive impairment.

Clients with dementia may have problems with:

  • Short-term memory
  • Keeping track of a dates and time
  • Paying bills
  • Identifying people
  • Planning and preparing meals
  • Remembering appointments
  • Running errands

Alzheimer’s

Alzheimer’s disease is responsible for 50% of dementia cases. Alzheimer’s is a progressive disease and not only affects memory but can often change the way people feel and act. Change can be stressful for anyone, especially for an Individual with Alzheimer’s disease. Alterations to environment can increase anxiety when trying to make sense out of an increasingly confusing world. Studies have shown that keeping individuals in their own environment and home can reduce behavioural symptoms.

The Care Team recommend home care being introduced in the early stages of Alzheimer’s disease, ensuring the client become familiar with the Carer.

Dementia Care Package

While coming to terms with Alzheimer’s diagnosis, you may be dealing with a whole range of emotions and concerns, our experienced home care Nurses and Community Liaison Officers will support and advise you on the road ahead.

The aim of dementia care is to restore and reduce the decline of Client’s functional ability. Developing an effective dementia homecare package is a diligent process. A comprehensive assessment of the Client’s needs is paramount. Dementia Care has to be personalised to the Client’s, environment, preference, behaviours and available support. Having structure in the daily routine is important, reducing anxiety and building familiarity. Our Team’s Nurses and Carers will establish routines to handle the most challenging times of day, such as evenings and night time.
Carers will engage in reminiscence therapy which is valuable to individuals in developing memory recall and intellectual exercise. Carers may use old photographs, music and activities as part of reminiscence therapy at home.

Communication plays a vital role in dementia care. We understand that Clients will experience changes and in some cases challenges when expressing and understand language. No one person has the same experience and changes can happen on a day-to-day basis. With you and your family’s input we will devise an effective plan to overcome these challenges. An effective care plan that changes as the Client’s needs change will ensure that the homecare package suits their requirements.

There are many changes that dementia can impact on. Clients can have variations in their behaviour. Some of these behavioural changes can be confusion, irritation and anguish. It can be difficult to know what to do or how to respond. It equally can be upsetting for family members to witness. Having education and understanding these kinds of behaviours and how to manage situations can alleviate some of the challenges. This process requires the right team to ensure that care is person centred.
Dementia can have an impact on sleep and cause night disturbance. Clients can become confused about the times of day and suffer with insomnia at night. There may come a time when the person gets up and dressed with the intention of going to work, the shops or to carry out some other errand. This phase can last for some time. Family members often find this period difficult to manage without support. Our Team can provide overnight care for you during this phase.
Our Team aim to keep continuity of care and reduce disturbances to the environment and routine. Dementia Care can be accessed as an hourly service or 24 live in care.
